Covid-19 Waiver -- this form must be read and agreed to complete registration.

I acknowledge the contagious nature of Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend preventative measures, including wearing masks (when not eating or drinking) and social distancing.

I further acknowledge that EnsembleIQ and the Path to Purchase Institute are relying on the Chicago HILTON in Chicago, IL to implement and maintain preventative measures to reduce the spread of Coronavirus/COVID-19.

I further acknowledge that EnsembleIQ and the Path to Purchase Institute cannot guarantee that I will not become infected with Coronavirus/Covid-19. I voluntarily seek to attend the P2PI LIVE and acknowledge that I am increasing my risk to exposure to the Coronavirus/COVID-19. I acknowledge that I must comply with all recommended procedures detailed on the event website, and shared at the event through signs and other means, as well as any state or local rules or guidelines in place at the event at that time.

I attest that:

• If I am experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills, repeated shaking with chills, muscle pain, headache, sore throat, or new loss of taste or smell within 10 days prior to the event start, I will not attend the event.
• If I believe I have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19 within 10 days prior to the event start date, I will not attend.
• If I am diagnosed with Coronavirus/Covid-19 and not yet been cleared within 10 days prior to the event start date as non-contagious then I will not attend the event.

I hereby release and agree to hold EnsembleIQ, the Path to Purchase Institute, and the Chicago HILTON in Chicago, IL, and all suppliers, harmless, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act by EnsembleIQ, or that may otherwise arise in any way in connection with attending and participating at P2PI LIVE. This liability waiver and release extends to the owners, partners, employees, contractors and suppliers retained by EnsembleIQ to produce P2PI LIVE. The foregoing is subject to change based on any Federal, state or local changes or recommendations.
